Should you wish to submit a claim to us, please click here to print our claim submission form.
Once the form is completed it may be faxed to us at 610.543.0461 or scanned and emailed to: jswesq@gmail.com. We will e-mail you an acknowledgment within one business day of receipt of the completed form and file the necessary proceedings if a Retainer Agreement is on file.

In addition to completing the claims form, please send a copy of the following documents:
- Statement of Account
- Contract
- Credit Application and/or Personal Guaranty
- Copy of Defendant's Check, if available
